The 5th Phase, Monitoring of the SLHCP, is the monitoring procedure, that will be set-up to follow the SLHCP implementation and periodically assess the actual evolution of the local H&C system, the advancement status of actions and the progress towards targets. For the monitoring procedure processing, a set of auditable, reportable, and verifiable key performance indicators (both quantitative and/or qualitative) will be set-up and regularly updated according to a well-defined frequency, in order to ensure its effectiveness, and based on the same approach and methodologies used for the set-up of the H&C baseline and the intervention scenarios.

Within the Plan4COLD project, several tools and software have been scouted and a selection of tools defined, mainly focusing on opensource, user-friendly and free to use ones. Please find below factsheet of selected tools that can be applied in Phase 5.
